(b) IN PROGRESS

(i)  Role of Chemo-radiation in Management of locally advanced Head and Neck Cancer Patients attending University College Hospital, Ibadan.

 

This study started  December, 2005, about 60% of patients present in advanced stage of disease with high tendency of metastasis. To improve local control and possibility of improving survival, chemo-radiotherapy  is being studied. Forty five consecutive patients  have been recruited into the

 Study, these patients received cisplatin 50mg/m2 every 21days for 4 courses while on  External Radiotherapy given daily to the total dose of 50Gy. Patients are being reviewed weekly, so far mild to moderate side-effects have been recorded. The outcome will be compared to earlier study done in the Department on patients that received only External Radiotherapy. 

(ii)  Evaluation of Pain and its relation to other symptoms of Advanced Cancer Patients at University College Hospital, Ibadan.

 

Pain is the most feared symptom of cancer patients, studies have shown that pain is usually under treated both in developed and developing countries. The aim of the study is to find out effect of pain on other symptoms, preliminary analysis of results of eighty patients, showed that presence of pain affected mostly symptoms such as headache, lack of sleep and appetite further analysis is being awaited to establish correlation.

(iii)  Role of Chemo-radiation in treatment of advanced primary Pediatric Brain tumours.


In advanced disease of pediatric patients there is a limit to the dose of radiation that could be given, hence Chemo-radiation is being considered to achieve synergistic effect in that the chemotherapy might increase the sensitivity of the tumor to radiation. Intra -thecal methotrexate is to be given weekly, while on radiotherapy. It is hopeful that that a good response will achieved to improve the quality of life of these patients

RESEARCH FOCUS

Clinical oncology is multidisciplinary in approach hence it involves Surgical Oncology, Gyneacological Oncology, Paediatric Oncology, Radiation Oncology, Genetic Therapy, Psycho-oncology and Medical Oncology.  My research focuses are on three main areas namely Paediatric Radiotherapy, Innovative Radiotherapy and Palliative Care of Advanced Cancer Patients. The main objective being towards solving problems encountered in treatment of numerous patients with limited facilities without compromising on effectiveness and safety.

Safe delivery of radiation to children requires immobilization and this we used to achieve by intramuscular paraldehyde by non Anaesthetist with minimal monitoring.  My research found out some complications which were avoidable, this led to the study of sedation in children with Ketamin while receiving Radiotherapy, with the good result obtained, Anaesthetists now supervise sedation of our Paediatric patients.

In view of rising population of cancer patients especially breast cancer, in collaboration with my colleagues research was done on effective dose delivery from our limited radiotherapy facility. Further studies revealed that patients with early disease and clinically negative axilla do well with only chest wall irradiation post surgery.   We can now treat more patients without compromising on disease free survival, achieving reduced morbidity especially lymphoedema of upper limbs.

Studies have confirmed that 70% of our patients come in advanced (stages iii & iv) diseases hence palliative care becomes an important issue.  My research showed that there is gross lack of knowledge about palliative care in our environment and there is no organized setting for palliative in Nigeria.  However the concept was well received by the terminally ill. To take care of pains especially pain of bony metastasis.  External radiotherapy with weak opiods was found to be effective in over 80% of patients since strong opiods are unavailable. Spinal metastases could be effectively treated with radiotherapy as observed in experiment conducted in paper no 8. Paper 9 showed the efficacy of combination Chemotherapy alone in an advance Non-Hodgkin’s lymphoma of intestinal tract complicated with an enterocutaneous fistula, the fistula healed up after 8 courses and patient in still alive 5 years after completion of this palliative regimen.

Research in collaboration with my colleagues found useful ondansetron to control emesis of Chemotherapy.  With the availability of generic ondansetron the cost is now affordable and it is now commonly used to control emesis of cancer chemotherapy.

My research work on palliative care in collaboration with my colleagues has brought together different professionals in the college of medicine and university college hospital with the name palliative care initiative of Nigeria and we have had two public awareness workshops at Ibadan.

This group will now oversee the newly commissioned Day Care Hospice in University College Hospital Ibadan. This is the first of its kind in Nigeria.

In view of my active involvement in  research,clinical services and tranning in Palliative care in Nigeria, I was recently elected as the National President of Hospice and Palliative Care Association of Nigeria. I am also a co-founder of the Association.

Researches

RESEARCH FOCUS

Clinical oncology is multidisciplinary in approach hence it involves Surgical Oncology, Gyneacological Oncology, Paediatric Oncology, Radiation Oncology, Genetic Therapy, Psycho-oncology and Medical Oncology. My research focuses are on three main areas namely Paediatric Radiotherapy, Innovative Radiotherapy and Palliative Care of Advanced Cancer Patients. The main objective being towards solving problems encountered in treatment of numerous patients with limited facilities without compromising on effectiveness and safety.

Safe delivery of radiation to children requires immobilization and this we used to achieve by intramuscular paraldehyde by non Anaesthetist with minimal monitoring. My research found out some complications which were avoidable, this led to the study of sedation in children with Ketamin while receiving Radiotherapy, with the good result obtained, Anaesthetists now supervise sedation of our Paediatric patients.

In view of rising population of cancer patients especially breast cancer, in collaboration with my colleagues research was done on effective dose delivery from our limited radiotherapy facility. Further studies revealed that patients with early disease and clinically negative axilla do well with only chest wall irradiation post surgery.   We can now treat more patients without compromising on disease free survival, achieving reduced morbidity especially lymphoedema of upper limbs.

Studies have confirmed that 70% of our patients come in advanced (stages iii & iv) diseases hence palliative care becomes an important issue. My research showed that there is gross lack of knowledge about palliative care in our environment and there is no organized setting for palliative in Nigeria. However the concept was well received by the terminally ill. To take care of pains especially pain of bony metastasis. External radiotherapy with weak opiods was found to be effective in over 80% of patients since strong opiods are unavailable. Spinal metastases could be effectively treated with radiotherapy as observed in experiment conducted in paper no 8. Paper 9 showed the efficacy of combination Chemotherapy alone in an advance Non-Hodgkin’s lymphoma of intestinal tract complicated with an enterocutaneous fistula, the fistula healed up after 8 courses and patient in still alive 5 years after completion of this palliative regimen.

Research in collaboration with my colleagues found useful ondansetron to control emesis of Chemotherapy. With the availability of generic ondansetron the cost is now affordable and it is now commonly used to control emesis of cancer chemotherapy.

My research work on palliative care in collaboration with my colleagues has brought together different professionals in the college of medicine and university college hospital with the name palliative care initiative of Nigeria and we have had two public awareness workshops at Ibadan.

This group will now oversee the newly commissioned Day Care Hospice in University College Hospital Ibadan. This is the first of its kind in Nigeria.

In view of my active involvement in research,clinical services and tranning in Palliative care in Nigeria, I was recently elected as the National President of Hospice and Palliative Care Association of Nigeria. I am also a co-founder of the Association.
