Biography:

Dr Olanrewaju Opeodu joined the University of Ibadan as a lecturer in September 2010 in the Department of Periodontology and Community Dentistry after the successful completion of his residency training.  He holds a master’s degree in dental science and is a fellow of the West African College of Surgeons specializing in periodontology. His main research focus is on periodontal medicine with passion for the inter-relationship between periodontal diseases and systemic conditions. We studied the effect of pregnancy on gingival health among some Nigerian women, during and after pregnancy, by assessing tendency to gingival bleeding on gentle probing. The finding was such that gingival health greatly improved at the end of pregnancy. The effect of periodontal disease on time to conception (TTC) among women attending fertility clinic was also studied, with the finding that good oral hygiene correlated with shorter TTC (< 1 year) compared with those women that had fair oral hygiene. Though not statistically significant, women of child-bearing age were encouraged to maintain good oral hygiene. We also reported an elevated level of matrix metalloproteinase-8 (MMP-8) among pregnant women which was linked with the possibility of increased prevalence of pre-term birth among Nigerians. MMP-8 is a pro-inflammatory mediator that had been implicated in pre-term premature rupturing of membranes (PPROM) in pregnancy and thus a preterm birth. MMP-8 is also elevated in periodontal disease, with the possibility of MMP-8 being a common risk factor for the two conditions. It was also discovered that subnormal sperm count was associated with periodontal disease compared with control, which made us to conclude that people of childbearing age should be encouraged to maintain good periodontal health, as this may be a factor in conception. Assessment of the periodontal condition and treatment needs of some pregnant women revealed that majority of them had gingival swelling, which significantly reduced at the end of pregnancy. There was a statistically significant relationship between the oral hygiene index and carotid artery intima media thickness and the blood pressure when the possible relationship between periodontal disease and hypertension was assessed. The reliability of past medical history as obtained by dentists while clerking patients and its implication in patients’ management in dental clinic was assessed by comparing patients’ history with an objective assessment of their blood pressure and blood glucose level. It was discovered that 4.4% of those that reported that they were not diabetic actually had elevated blood glucose level, while there was a co-existing elevated blood glucose level and diastolic blood pressure among 3.0% of the patients. These conditions, despite not previously known, may present as medical emergencies in dental clinic either before, during or after treatment. Therefore, practitioners were encouraged to look out for both conditions and the possibility of their co-existence in the same patient. We recommended that adult patients should have their vital signs checked before treatment, while blood glucose assessment should be done for patients above 45 years before tooth extraction. This has become routine evaluation in our clinics.
